Application for Passport by Mail

Requirements: Must have been over sixteen when current passport was issued. Current passport must have been issued within last fifteen years. Passport cannot be in mutilated condition.

The passport which is to be renewed (it will be cancelled and returned to you except in extenuating circumstances
1 Original DS-82 "Application for Passport by Mail" completed online printed on two separate sheets of paper (applications completed on one sheet double sided are not acceptable)and signed in ink. Applications that are not completed this way cannot be accepted through visa services by the passport agency
2 Passport Quality Photographs
Letter of Authorization
Copy of Tickets/Itinerary
Cover Sheet

NOTE: All instructions on the passport application and attached pages must be read. If the U.S. Passport agency feels they have not been read they will not issue a passport.

Fees and Processing times:

Expedited Processing: $135.00 (Due to changes in passport processing procedures we will no longer be able to submit non-expedited passport applications.)

Expert Visa Services, Inc. Service Fees(charged in addition to the U.S. Passport Agency's fees):
Processing in 6 days and longer: $30.00
Processing 5 days and under: $45.00 when available
Processing is based on Departure Date evidence provided by applicant
U.S. Government fees and Return Charges are not included in Expert Visa Services fees

Due to overwhelming demand for passports minimum processing time is 2 weeks. If you need a passport in less than 2 weeks contact the closest passport agency.
