The U.S. Passport agency is no longer accepting amendment applications.

To change the name in a passport, correct information, or extend the validity of a passport issued within then last year please read below, if you need to change any information in a passport over 1 year old please see the section for Application for Passport by Mail

To change information in a passport less than 1 year old submit the following information:

• 1 DS5504 Passport application form Original completed online, printed and signed in ink. Hand written applications are not accepted by the Chicago Passport Agency.
• Most recent Passport
• 2 U.S. Passport Quality Photographs
• Proof of name change (Marriage certificate, original or certified copy or court order showing name change.)
• Proof of travel if passport is needed in an expedited manner (itinerary or copy of airline ticket)
• Cover Sheet
• Letter of Authorization

Due to overwhelming demand for passports minimum processing time is 2 weeks. If you need a passport in less than 2 weeks please contact your closest passport agency

For expedited processing (passport needed in less than 4 weeks) an expedite fee of $60.00 will be charges by the U.S. passport agency, this does not include Expert Visa service charges. (Due to changes in passport processing procedures we will no longer be able to submit non-expedited passport applications.)

Expert Visa Service charges:

Processing in 6 or more working days:$30.00

Processing in 5 or less working days:$45.00 when available

The U.S. passport agency determines all passport processing times and bases the processing time on the proof of travel provided. In most cases we will be able to meet the dates a traveler wants or needs a passport but final decision lies with the U.S. passport agency.
