• -1 Completed Visa Application Application must be typed. No block can be left blank, if the block is empty or does not apply to you application type n/a. The attached application is not the New York visa form, it is the Washington DC Chinese visa application)
• Supplemental form for Non US Passport Holders, "Z" visa applicants and Student Visas-must be typed
• -U.S. Passport Quality Photograph (Color Photographs Only)
• -Copy of Ticket/Flight Itinerary (recommended)
• -Cover Sheet
• If Staying with friends or family members in China a faxed letter of invitation is required. (Must be faxed no email letters will be accepted.)

The Chinese visa can be valid for single, double, 6 month multiple or 1 year multiple entries. The number of entries issued on the visa is determined by what is on the official Government invitation from China. The number of entries cannot be changed once an application is in process. Non U.S. passport holders cannot get multiple entry visas at this time.
