Chicago visa application and Applicant's Sworn Statement Waiver Consulate General of Brazil in Chicago has jurisdiction for:
Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, Nebraska, North Dakota, South Dakota, and Wisconsin. The application for Chicago's jurisdiction is typed on Brazilian web site (follow the link above for the Chicago application) after the application has been completed press the button to send the application and a page will come up to be printed and signed. Please note failure to complete information on the form will result in delay of processing your application please be sure that all fields marked with an * are completed in addition all fields pertaining to you application should be completed as well ex: Profession, Employer name and address, contact in Brazil name and address .
 

New York visa application Consulate General of Brazil in New York has jurisdiction for: Connecticut, Delaware, New Jersey, New York, Pennsylvania, and the Island of Bermuda

Washington DC visa application Consulate General of Brazil in Washington DC has jurisdiction for: Kentucky, Maryland, Ohio, Virginia, West Virginia, and the District of Columbia Note: Will require a photocopy of applicants drivers licence.

Houston visa application Consulate General of Brazil in Houston has jurisdiction for: Arkansas, Colorado, Kansas, Louisiana, New Mexico, Oklahoma and Texas

 

San Francisco visa application and Applicant's Sworn Statement Waiver Consulate General of Brazil in San Francisco has jurisdiction for: Oregon, Washington, Alaska and in California over the following counties: Alameda, Alpine, Amador, Butte, Calaveras, Colusa, Contra Costa, Del Norte, El Dorado, Fresno, Glenn, Humboldt, Inyo, Kings, Lake, Lassen, Ladera, Marin, Mariposa, Mendocino, Merced, Modoc, Mono, Monterey, Napa, Nevada, Placer, Plumas, Sacramento, San Benedito, San Francisco, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Shasta, Sierra, Siskiyou, Solano, Sonoma, Stanislau, Sutter, Tehama, Trinity, Tulare, Tuolumne, Yolo, Yuma.

Miami visa application. Consulate General of Brazil in Miami has jurisdiction for: Florida, Puerto Rico, U.S. Virgin Islands
Los Angeles visa application. Consulate General of Brazil in Los Angeles has jurisdiction for: Arizona, Hawaii, Idaho, Montana, Nevada, Utah, Wyoming, U.S. Minor Outlying Islands in the Pacific (Johnston, Midway, Wake, Howland, Jarvis, Baker, Palmyra and Kingman), and in the following counties in California: Imperial, Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Luis Obispo, Santa Barbara, and Ventura.
Boston Visa application. Consulate General in Boston has jurisdiction for: Massachusetts, New Hampshire, Rhode Island, and Vermont (We have no representation in Boston, please complete the New York visa application form)

Atlanta visa application. Consulate General of Brazil in Atlanta has jurisdiction for: Georgia, Alabama, North Carolina, South Carolina, Mississippi, Tennessee

Note: Atlanta requires two complete and signed visa application forms and two U.S. Passport quality photographs, notarized U.S. company letter completed in duplicate and invitation letter from Brazil in Portuguese.
